| Permission to Administer Medication Form Filling out the top portion of the Permission to Administer Medication Form will give CSEC’s school nurse permission to administer over-the-counter medication to the student when needed. The bottom portion is for prescription medications. | ||

Documents Required
Copy of Birth Certificate
Immunization Records (Must be signed by doctor or nurse.)
Special Ed Documents, if applicable (You may seal these in a confidential envelope if you wish.)
IEP for Special Education Services (Must include last eligibility meeting documents, consent for placement forms, permission to assess form, current IEP, etc. See page 3 of the enrollment form.)
504 Plan, if applicable (Must include 504 Plan for accommodations with official diagnosis.)
Gifted/Talented documents, if applicable (Must include GT-identification assessments and Advanced Learning Plan.)
Homeschool Grade Reports must be provided by parents for their homeschooled students including a transcript ( for incoming 10th-12th graders) or an 8th Grade Report Card (for incoming 9th Graders) Homeschool Transcrips must include original dated signature of parent/guardian, student full name and date of birth, grade level and school year each course was completed and credits awarded for each course based on % of course completed or Carnegie Units (120 hours=1 year credit).
