  Permission to Administer Medication Form    Filling out the top portion of the Permission to Administer Medication Form will give CSEC’s school nurse permission to administer over-the-counter medication to the student when needed.  The bottom portion is for prescription medications.

Documents Required

Copy of Birth Certificate   

Immunization Records   (Must be signed by doctor or nurse.) 

Special Ed Documents, if applicable   (You may seal these in a confidential envelope if you wish.) 

IEP for Special Education Services   (Must include last eligibility meeting documents, consent for placement forms, permission to assess form, current IEP, etc.  See page 3 of the enrollment form.) 

504 Plan, if applicable       (Must include 504 Plan for accommodations with official diagnosis.)

Gifted/Talented documents, if applicable     (Must  include GT-identification assessments and Advanced Learning Plan.)

Homeschool Grade Reports must be provided by parents for their homeschooled students including a transcript ( for incoming 10th-12th graders) or an 8th Grade Report Card (for incoming 9th Graders)  Homeschool Transcrips must include original dated signature of parent/guardian, student full name and date of birth, grade level and school year each course was completed and credits awarded for each course based on % of course completed or Carnegie Units (120 hours=1 year credit).  An optional template is available at http://csec914.org/prospective-students/enrollment-process.    

 

 

 

CSEC shares a campus with CTU and partners with CSEC to offer CTU college courses to CSEC students as part of  CSEC's normal school day.

         

  CSEC is authorized by Charter School Institute, a state-wide organization that acts somewhat like a school district.

           

Colorado League of Charter Schools supports CSEC with training and other services.
            

Many CSEC students attend off-campus classes at Pikes Peak Community Collegeand at University of Colorado at Colorado Springs
